BONUS: Why Delivery Breaks Down in Growing Tech Organizations—Ownership, Guardrails, and the People-First Company With Prashanth Tondapu

What actually slows down tech teams—lack of talent, or lack of ownership? In this episode, Prashanth Tondapu shares lessons from leading through global-scale failures, scaling from a small team to a 100-person company, and discovering why guardrails beat rigid processes when it comes to building teams that own outcomes and execute with discipline.

Diffusion of Accountability: When Everyone Is Responsible, Nobody Is

“Crisis is not the problem. Crisis is the one that uncovers the problem that has always existed.”

Early in his career, Prashanth witnessed a large-scale failure at a major technology company—not because the team lacked talent, but because accountability had become diffused. When too many people are responsible for something, it translates to nobody being responsible. The team was brilliant individually, but there was no clear demarcation of who owned what outcome. On good days, everything worked. But when things went wrong, there was no single person who could no longer delegate accountability to someone else. Prashanth connects this to the concept from Extreme Ownership by Jocko Willink—while the whole team must feel ownership, each individual needs laser-focused clarity on their specific accountability. As he frames it: outcome can only come with 100% emotional commitment to a particular problem, and when five people share that commitment, each carries only 20%. That’s where breakdowns happen.

The Leadership Design Problem: From Computers to People

“I was a developer who imagined that humans are also going to be as predictable as computers. Until 6 or 7 people, it works well because you can be everywhere. But as soon as we increased above 7, I was not able to be everywhere.” Continue reading BONUS: Why Delivery Breaks Down in Growing Tech Organizations—Ownership, Guardrails, and the People-First Company With Prashanth Tondapu

How Facebook Scaled Psychological Safety – Mehmet Baha Shares Their Journey

This is a guest blog post by Mehmet Baha, who authored Creating Psychological Safety at Work. In this blog post, Baha explores his story and journey at Facebook and shares his lessons on establishing a culture (and more) that leads to psychological safety at work.

“Our international company is going through a transformation…We have a company event and we are considering inviting you as a speaker on the topic of psychological safety.” was written in the email. I was excited about the opportunity and replied immediately to arrange an online meeting.

In that online meeting, one of the senior leaders of the organization said, “We already have some knowledge about psychological safety. We did a few workshops on the topic. However, we do not know if there are organizations with high levels of psychological safety. If yes, what do they do to achieve this?” I get the same question from other business leaders from time to time.

My Experience at Facebook: A Case Study in Psychological Safety

I was lucky to be one of the first Facebook employees in Europe where I contributed to its success from 2009 until 2013. Across those years, Facebook was growing exponentially and there was a high degree of psychological safety. Of course, Facebook wasn’t paradise; it had its own challenges.

However, a substantial number of employees felt safe to express their questions, concerns, ideas, and make mistakes. It was a workplace of innovation and high employee engagement. Here you can see a photo of Mark Zuckerberg talking with a group of Facebook employees in Dublin, Ireland. I am the person at the far left of the photograph, directly next to him.

Although Facebook and its senior leaders did not use the phrase “psychological safety” at that time, they had in fact created a workplace of psychological safety. To understand how Facebook then scaled psychological safety, we need to understand three aspects of an organization: culture, power, and strategic design created by Massachusetts Institute of Technology (MIT)(1).

Culture: The Foundation of Psychological Safety

CTO Series: Engineering Leadership, Automation, and Trust with Dan Hollinger

In this CTO Series episode, we sit down with Dan Hollinger, an accomplished engineering leader passionate about fostering empathy, transparency, and trust in tech teams. Dan shares pivotal lessons from his career, from building scalable automation systems to navigating complex leadership challenges. We cover key strategies for aligning tech initiatives with business goals, fostering collaboration, and ensuring long-term technical health.

Defining Leadership Through Automation and Empowerment

“Enable your humans to focus on the interesting work—automation should take care of the rest.”

Dan recounts his transformative experience at CCP Games, makers of EVE Online, where a robust test automation system changed his perspective on scaling technical processes. This role introduced him to the power of automation in freeing up engineers to focus on more exploratory and impactful tasks. He emphasizes how empowering self-directed teams with high-level vision statements enables creativity and innovation.

Building Self-Correcting Processes

“Always retro your processes—don’t let them run on autopilot.”

Dan explains the importance of self-correcting processes, using the SEV (Side Event) system as an example. He highlights how retrospectives can improve response times and prevent future crises. For Dan, consistent reviews are the key to maintaining agile, resilient systems that adapt to evolving needs.
